Emergency Contact Form
Must be filled out every year – is yours completed yet?
Emergency Contact Forms are required for everyone at Wedgewood. Having these forms on hand helps us in the event of an emergency. Members will not be allowed to register for tennis and swim teams or lessons until their completed form is on file.
Again, each member (adults and children in your family) and all registered house guests must have an emergency contact form completed for the summer. Please read through each question and make sure it is completed for the individual person you are completing it for. All members who are under the age of 21 will need to provide parent contact information and additional emergency contact information. All members who are over the age of 21 will need to provide emergency contact information. At the end of the form you will have an opportunity to complete additional forms for all of the members of your household.
